For the participation request to the Courses it is necessary to fill up the following “PERSONAL DATA” PARTICIPATION REQUEST FORM. At its reception the agreement with the courses partecipation conditions 'Seaman's Training Course Agreement' and 'Order of Payment' will be sent to the applicant single seaman and 'Seafarer's Service Agrement' to the applicant Owners or Manager or Operator or Bare Boat Charterers or Manning Agent.
In case of confirmation the respective agreement must be returned countersigned for acceptance by each applicant by fax or attached by e-mail. |
